Detailed Information for Home DSL

DSL pricing is composed of 4 components:
    1. ISP charges from Mainstream
    2. DSL line charges from the telephone company (typically Qwest)
    3. DSL modem charges (one time charges)
    4. Setup and installation charges (one time charges)

1. ISP charges from Mainstream:
    - $5.00/month credit for the first 6 months
    - $9.25/month when paid on an annual basis
         ($81 first year, $111 subsequent years)
    - $9.95/month when paid on a semi-annual basic
         ($29.70 first 6 months, $59.70 following 6 months)
    - $11.95/month when paid on a monthly basis ($6.95 for first 6 months)

2. DSL line charges: (Through February 23rd, 2008)
    - 256K: $15/month
    - 1.5MB: $25 for the first year, $28/month thereafter
    - 3/5mb: $35 for the first year, $38/month thereafter
    - 1.5mb Standalone: $30/month for the first year, $33/month thereafter
    - 3/5mb Standalone: $40/month for the first year, $43/month thereafter
    - Price for Life offer will keep your 1.5mb or higher speeds at the first
      year prices. This is a 2 year commitment and if you terminate before 2 years
       Qwest will charge you a $200 early termination fee.

3. DSL modem charges: (one time charges).  Pick one of the following:
    1. Actiontec M1000 modem
        - Rental $5/month or Purchase: $59.99 from Qwest
        - Shipping/handling $9.99
        - 1 Ethernet port
        - Optional Wireless module $19.99 plus $5 shipping/handling
    2. Actiontec Wireless Starter kit (model GS583AD3-08).
        - Purchase: $89.99 from Best Buy
        - 4 Ethernet ports and Wireless capability

 4. Setup and installation charges
     - Qwest DSL line activation fee of $99 is waived through ??? 2008
     - Installation options:
